wrong info on enrollment form

If I have one incorrect information on the enrollment form, is my policy valid?This is for a term life no medical exam.

I was reviewing the enrollment form that came along with my policy and it had five health questions and on one of them was mark as no when it probably should have been yes.

Should I call the insurance company and let them know. What will happen. Is there a penalty?
